Door window winder - what's going on here + cure?

I'm having severe problems with my passenger door window. On winding, the entire inner door shell flexes and the winding arm moves out rather than transferring the movement to up and down. This results in a lot of creaking and banging and in some cases the mechanism actually hits the other inner parts of the door and jams. I've replaced the winder mechanism twice, the glass assembly, the pivot bolt, and anything else I can think of but it's as if the winding arm wants to move sideways rather than across and so the door frame just flexes. The glass slides up and down freely but the mechanism uses all it's power to flex outwards so the glass hardly moves. I know the pivot bolt has worn the hole slightly oval, but the resulting movement seems way out of proportion, and I'm on my third winder mechanism so they can't all be wrong. I know the central brace strut is missing; I've removed that to get a better view of things but when it's fitted, the arm catches on it anyway. Any thoughts?
